Steps to Apply for an Intervention at Twelve Stones
- Call TS office to ask any questions and review your situation with us to see if we are a good fit.
- Print the Complete Application off the website or call TS and one can be mailed to you.
- Fill out the counselee portion of the application (Consent to Counseling, Authorization to Release Information, Agreement to Participate, Personal Data Inventory).
- Prayerfully consider and then locate an advocate (if you are having trouble, please speak to your pastor or other church leaders who may be able to assist you in locating an advocate).
- Once you have located an advocate, provide them the advocate portion of the application or have them print it off the internet (Advocate Information Letter and Advocate Form).
- Provide your pastor or his designate the Pastor Form to fill out or have him print it off the internet.
- Mail in or fax the counselee portion of the application to TS along with the $50 application fee. Follow up to make sure that your advocate and pastor mail in their portion as well.
- Once all the information has been sent in, please call the Ministry Coordinator, Rhonda Garrison to finalize available dates.
- Mail in a $500 deposit to hold your dates.
- This deposit will be deducted from your final amount due. An invoice will be provided to you sometime during your stay.
- Payment arrangements are available if you need additional time to pay the balance.
- If finances are a concern, please contact your church leaders who may be able to assist you. Many churches are willing to assist those in their congregation who need financial assistance.
- Benevolence funds are available on a case by case basis so please contact our Ministry Coordinator Rhonda Garrison if finances are hindering you from coming.
